Applying for a BASK service dog:  

Please fill out this entire online application along with:

– Sealed letters of reference and diagnosis from each of your physicians, therapists, and counselors stating that your disability is work related or service connected and that a service dog is medically necessary and will be beneficial to you.  

– Please provide a CORI on yourself and include it with your application packet.

– Applicants who have been discharged from the military must provide proof of honorable discharge and a copy of their DD214 form.   

– Include a letter of recommendation (from a non-family member).

The Process:

Upon receipt of your email letter of interest, a BASK representative will call you for a telephone interview.  If that goes well, you will receive the application email. When we get the packet back from you, the BASK board of directors client review panel will review your application and if appropriate, will call or email you to set up an interview to be held in Medway, MA. All members of the household should be present at the initial interview. This usually lasts 60-90 minutes. You will be able to ask any questions you may have at this time or at any time before or after.  If all goes well, a 60-90 minute home visit will be scheduled. All members of the home should be present at that time. Two or more BASK representatives will ask you, your family and other household members additional questions to better understand your needs and your living environment. BASK will look at every room in the house as well as any/all yard spaces. BASK will meet any other pets. A BASK assistance dog will also be present.

If at any time, you feel that a service dog is not right for you, please let us know and we will remove your application from our files or place it on hold until you’re ready.  If the client review board feels that BASK is able to provide an assistance dog for your particular case, we will contact you to let you know that you have been selected as a client.

Please note that the client review board meets once a month and the time frame of this process varies and may take several months.
